By applying various perspectives and asking sharper <a href=\"https:\/\/mgrush.com\/blog\/how-to-develop-questions\/\" target=\"_blank\" rel=\"noopener\">questions<\/a>, you can lead a team to innovation.<\/h2>\n<p><span style=\"font-size: 14pt;\">Named after Professor <a href=\"http:\/\/history-biography.com\/kaoru-ishikawa\/\" target=\"_blank\" rel=\"noopener noreferrer\">Kaoru Ishikawa<\/a> (University of Tokyo), he developed the root cause analysis method in 1945 to resolve steel production problems. Also known as &#8220;Fishbone&#8221; diagrams, they support analysis, identify gaps, and provide insight into possible <a title=\"SMART Versus DUMB Criteria\" href=\"https:\/\/mgrush.com\/blog\/smart-measures\/\" target=\"_blank\" rel=\"noopener noreferrer\">SMART criteria<\/a> (i.e., Specific, Measurable, Adjustable, Relevant, and Time-Based). Stress leading indicators, rather than lagging indicators. Workshop steps to <a href=\" https:\/\/mgrush.com\/blog\/scope-creep\/\" target=\"_blank\" rel=\"noopener noreferrer\">facilitate<\/a> root cause indicators and build a simple cause and effect diagram include:<\/span><\/p>\n<ol>\n<li><span style=\"font-size: 14pt;\">In advance, prepare a blank fishbone drawing (devoid of content) using either multiple sheets of paper or some professional <a href=\"https:\/\/mgrush.com\/blog\/meeting-graphics\/\" target=\"_blank\" rel=\"noopener noreferrer\">drawing<\/a> tool.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Use the objectives of the project to identify the \u201cprimary effect\u201d or result that needs to be changed or accomplished. Decide on a phrase or a one-word label that captures the <a href=\"https:\/\/mgrush.com\/blog\/facilitate-meaning\/\" target=\"_blank\" rel=\"noopener noreferrer\">meaning<\/a> of the end state. However, as a practical activity, you may also focus on fewer, even one or two primary perspectives, or four as illustrated above.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Alternatively, you may launch a <a href=\"https:\/\/mgrush.com\/blog\/facilitate-innovation\/\" target=\"_blank\" rel=\"noopener noreferrer\">brainstorming<\/a> activity of all possible causes, and then utilize a common purpose to help the team categorize them. Considering the Pareto Principle, many leaders approach their cause-and-effect diagrams with four high-impact perspectives. For example, study the perspectives below to isolate the ones most likely to have an impact. Four perspectives in the illustration include:<\/span><\/li>\n<\/ol>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-size: 14pt;\"><strong>Tools<\/strong>: Traditionally seen as the technology or equipment that leads to error, but could also reflect tangible resources that provide possible causes<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Method<\/strong>: Isolates the activities or tasks that might be the source of concern or the opportunity for improvement<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>People<\/strong>: Intends to capture the group relationships and quality of decisions made<\/span><\/li>\n<li><span style=\"font-size: 14pt;\"><strong>Data<\/strong>: Traditionally seen as the information required by the causal element to create the desired effect<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3>Use Breakout Teams for Root Cause Analysis<\/h3>\n<p><span style=\"font-size: 14pt;\">You might consider using breakout teams and assigning one or more primary perspectives or potential causes to each team. Strive to confirm the most likely cause as a large group before breaking out. Then assign different causes or perspectives to each team, or have them work offline for detailed development.<\/span><\/p>\n<ul>\n<li><span style=\"font-size: 14pt;\">During a break, lunchtime, or evening, create an illustration of your diagram. Additionally, provide your <a href=\"https:\/\/mgrush.com\/blog\/better-listeners\/\" target=\"_blank\" rel=\"noopener noreferrer\">workshop<\/a> <a href=\"https:\/\/mgrush.com\/blog\/selective-perception\/\" target=\"_blank\" rel=\"noopener noreferrer\">participants<\/a> with full narrative definitions for each of the perspectives used\u00a0in your fishbone diagram.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Depending on time constraints, lead your root cause analysis activity either by beginning with the most important perspective, taking the likely causes within a perspective, or perhaps grabbing the easiest to manage, the \u201clow-hanging\u201d fruit. Determine <a href=\"https:\/\/mgrush.com\/blog\/group-clarity\/\" target=\"_blank\" rel=\"noopener noreferrer\">clear<\/a> and simple <a href=\"https:\/\/mgrush.com\/blog\/power-of-questions\/\" target=\"_blank\" rel=\"noopener noreferrer\">questions<\/a> in advance and know what you intend to do with the results. Understand the type of <a href=\"https:\/\/mgrush.com\/blog\/meeting-documenter\/\" target=\"_blank\" rel=\"noopener noreferrer\">documentation<\/a> required to satisfy your deliverables. Once your participants fully understand the question, agree on the causes associated with each perspective. Their consensual understanding makes it easier to build consensus around <a href=\"https:\/\/mgrush.com\/blog\/different-priorities\/\" target=\"_blank\" rel=\"noopener noreferrer\">priorities<\/a> and next steps. Carefully identify <a href=\"https:\/\/mgrush.com\/blog\/action-plan\/\" target=\"_blank\" rel=\"noopener noreferrer\">WHO does WHAT by WHEN<\/a> to design your next steps.<\/span><\/p>\n<h3><strong>Changing Perspectives for Root-Cause Analysis<\/strong><\/h3>\n<blockquote><p><span style=\"font-size: 14pt;\"><strong><em>NOTE<\/em><\/strong><em>: Use any of the <strong>perspectives<\/strong> suggested below and combine perspectives from different categories, or make up your own perspective to help your group focus their input from a specific <strong>point of view<\/strong>. Identify potential root causes within each primary area or perspective. Borrow liberally from the five perspectives\u00a0below listing 30 potential causes:<\/em><\/span><\/p><\/blockquote>\n<h4 style=\"padding-left: 40px;\"><span style=\"font-size: 14pt;\"><strong><em>The 6 M\u2019s Category<\/em><\/strong><\/span><\/h4>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-size: 14pt;\">Machines, Manpower. For example, ask&#8230; &#8220;What are the possible machine issues affecting\/ causing&#8230;?&#8221;<\/span><\/p>\n<ul>\n<li><span style=\"font-size: 14pt;\">Repeat this procedure with each perspective to produce potential causes. Continue asking, &#8220;Why is this happening?&#8221; and put additional causes against each perspective.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Exhaust each perspective until you no longer get useful information as you ask, &#8220;Why is that happening?&#8221;<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Analyze the results of the fishbone after team members agree that an adequate amount of detail has been provided under each major perspective. Look for those items that appear in more than one perspective. These repetitive factors become highly likely as frequent causes that will demand more time and generate longer discussions.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">For those factors identified as the &#8220;most likely causes,&#8221; use a <a href=\"https:\/\/mgrush.com\/blog\/facilitate-prioritization\/\" target=\"_blank\" rel=\"noopener\">prioritization<\/a> method to lead the team to a <a href=\"https:\/\/mgrush.com\/blog\/build-consensus\/\" target=\"_blank\" rel=\"noopener noreferrer\">consensus<\/a> about listing those factors in sequential priority, with the first factor being the \u201cmost probable&#8221; or \u201cmost impactful\u201d cause. For a simple and highly effective prioritization method:<\/span>\n<ul>\n<li><span style=\"font-size: 14pt;\">Build the criteria for evaluation.<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Separate the <a href=\"https:\/\/mgrush.com\/blog\/smart-measures\/\" target=\"_blank\" rel=\"noopener noreferrer\">SMART<\/a> from the fuzzy (where SMART discussed elsewhere equates to Specific, Measurable, Adjustable, Relevant, and Time-Based as compared to <a href=\"https:\/\/mgrush.com\/blog\/smart-dumb\/\" target=\"_blank\" rel=\"noopener noreferrer\">DUMB<\/a> which equates to Dull, Ubiquitous, Myopic, and Broad).<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Prioritize or rank the criteria using <a href=\"https:\/\/www.youtube.com\/watch?v=sJd9s9UK1Dc\"><em>PowerBalls <\/em><\/a>and<em><a href=\"https:\/\/mgrush.com\/blog\/bookend-method\/\" target=\"_blank\" rel=\"noopener noreferrer\"> Bookends<\/a>.<\/em><\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Appealing to the criteria, helps the group identify the most impactful of the \u201cmost likely causes.\u201d<\/span><\/li>\n<li><span style=\"font-size: 14pt;\">Where the group remains uncertain, <a href=\"https:\/\/mgrush.com\/blog\/selective-perception\/\" target=\"_blank\" rel=\"noopener noreferrer\">challenge<\/a> any fuzzy factors to create understanding, but only let them use the fuzzy factors when discussing critical causes.
